Job summary

UNITAS is recruiting a Clinical Pharmacist Specialist for a hospital setting. You will lead patient-centered medication therapy management, collaborate with physicians, nurses, and other clinicians, and ensure safe, effective, and timely medication services across clinics.

You will design stewardship programs, oversee Pharmacy and Therapeutics processes, and ensure compliance with URAC, HIPAA, and regulatory standards while supporting education and training for health care providers.

Qualifications

• Graduate of an accredited College of Pharmacy.
• Doctor of Pharmacy (PharmD) or other advanced degree (highly preferred).
• Completion of an accredited Pharmacy Residency (highly preferred).
• Clinical pharmacy experience (1+ years).
• Knowledge of contemporary clinical practice and service.

About the role

• The Clinical Pharmacist Specialist is responsible and accountable for the provision of safe, effective, and prompt medication therapy for the hospital. Reporting to the Chief Pharmacy Officer, this role delivers direct, patient-centered care and integrated pharmacy operational services in a medical home practice setting alongside physicians, nurses, and other medical personnel. Aligned with target interdisciplinary programs, the Clinical Pharmacist Specialist provides medication therapy management within patient care services, ensures pharmaceutical care programs are appropriately integrated across the hospital\'s clinics, and serves as a departmental resource and liaison to other departments while providing education and training to health care providers.

What you\'ll do

• Lead assigned patient service lines, clinical areas, and therapeutic programs, ensuring drug utilization aligns with patient needs, evidence-based best practices, and compliance and regulatory standards (including URAC).
• Deliver direct, patient-centered care—assessing needs, tailoring therapy and education to patient age and condition, and providing accurate, safe, timely, and appropriate medication therapy while helping expand pharmacy services.
• Complete critical patient monitoring and chart review to detect, prevent, and mitigate drug-related problems (e.g., improper drug or dose selection, adverse reactions, drug interactions, treatment failures); identify high-risk patients and intervene to improve adherence, outcomes, and safety.
• Provide MTM services through the hospital\'s preferred platform and collaborate with the multidisciplinary team, communicating with providers and caregivers to make evidence-based recommendations and ensure continuity of care.
• Design and implement stewardship, restriction, and surveillance programs, and provide oversight of the Pharmacy and Therapeutics Committee—including Antimicrobial Stewardship, Continuous Quality Improvement, and participation in the URAC Quality Management Committee.
• Leverage the 340B Drug Pricing Program to maximize clinical outcomes for patients.
• Review, prepare, dispense, compound, and document pharmaceutical orders and products per state and federal regulations, maintaining proficiency in EMR and pharmacy systems.
• Track and benchmark program performance for operational, quality, and financial efficiency—monitoring business metrics and KPIs, reviewing pharmacists\' clinical activity and financial-impact documentation, and developing reporting on clinical pharmacy claim data for stakeholders and committees.
• Generate and maintain documentation for clinical program audits and business processes, and identify, design, and implement medication-use system improvements to advance safety, outcomes, and cost control.
• Serve as a liaison between operations, providers, drug manufacturers, and PBMs—coordinating clinical requirements for manufacturer programs and helping leadership and stakeholders understand the impact of clinical business decisions.
• Partner with the Chief of Pharmacy to identify opportunities, develop strategic plans, and oversee project implementation, and precept students, residents, and staff to advance practice.
• Support the hospital\'s mission, vision, and values and comply with all policies, state and federal regulations, URAC requirements, and HIPAA.
