Position Summary:

Coordinate all systems/services required for an organized, multidisciplinary, patient-centered care team approach, and assure quality, cost-effective care for the identified patient population. Manage the course of treatment of patients, coordinating care with physicians, nurses, and other staff to ensure quality patient outcomes within established time frames and with efficient resource utilization. Conduct assessments, initiate disease management protocols, determine and manage outcomes, ensure continuity of care through discharge planning, resource utilization, and variance analysis. Serve as a contact for patients, families, healthcare team members, community resources, and employees as needed. Ensure adherence to hospital and departmental policies and procedures. Patient care may include neonate, pediatric, adolescent, adult, and geriatric groups.

Detailed Responsibilities:
  1. PATIENT CENTERED MED - Promote core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice.
  2. IDENTIFICATION - Identify appropriate patients requiring case management interventions using established procedures.
  3. DATA - Perform assessments, collect, review, and analyze information collaboratively with patients and healthcare team members.
  4. ASSESSMENT - Evaluate the patient's clinical and psychosocial status and current treatments.
  5. NEEDS - Assess patient/family needs related to diagnosis and treatment, providing options and resources.
  6. ORDERS AND REFERRALS - Obtain necessary orders and coordinate referrals for home health, oxygen, and equipment.
  7. REPORTS & RECORDS - Maintain tracking systems and compile reports and records.
  8. COLLABORATION - Build relationships with other departments and community agencies to support quality care.
  9. GOALS - Assist in setting patient-centered goals collaboratively.
  10. PLAN OF CARE - Develop comprehensive multidisciplinary care plans.
  11. DISCHARGE PLANNING - Conduct timely discharge planning in collaboration with the team.
  12. VARIANCES - Intervene when variances from treatment plans occur.
  13. RESOURCES - Coordinate and evaluate resource use for quality and cost-effectiveness.
  14. INTERVENTIONS - Monitor patient responses to interventions.
  15. VARIANCE - Review and resolve care protocol variances.
  16. TREATMENT CONFERENCE - Participate in interdisciplinary evaluations.
  17. EDUCATION - Provide patient and family education based on needs.
  18. INFORMATION - Develop communication activities for the healthcare team.
  19. CONTINUITY OF CARE - Ensure seamless care across settings.
  20. MEETINGS - Attend team and departmental meetings.
  21. CARE PLAN - Incorporate interdisciplinary recommendations.
  22. COMMUNICATION - Use effective communication strategies.
  23. DOCUMENTATION - Document assessments and progress timely.
  24. QUALITY - Engage in quality improvement activities.
  25. COMMITTEES - Represent the department in meetings and committees.
  26. DEVELOPMENT - Pursue ongoing professional growth.
  27. PATIENT SAFETY - Follow safety policies, report concerns, and promote safety culture.
  28. MEDICATION - Administer medications following safety protocols (UPC and CPC only).
  29. PATIENT CARE - Write treatment plans, coordinate care, and facilitate education (UPC and CPC only).

Education:

  • Essential: Program Graduate
  • Nonessential: Bachelor's Degree

Specialization:
Essential: Nationally Accredited Nursing Graduate
Nonessential: Nursing

Experience:

  • Essential: 1 year of directly related experience
  • Nonessential: Bilingual in English and Keres, Tewa, Tiwa, Towa, Zuni, or Navajo

Credentials:

  • Essential: RN license in NM or reciprocal agreement
    CPR for Healthcare/BLS Provider or Professional Rescuer within 30 days

Physical Conditions: Light Work: Exerting up to 20 pounds occasionally, up to 10 pounds frequently, and negligible force constantly. May require walking or standing; may involve pushing/pulling.

Working Conditions: Minor hazards such as physical risks, dirt, dust, fumes, noise.
