Job summary

Empath Home Health, a member of Empath Health, seeks a compassionate PRN Home Health PTA to join our home health team delivering individualized care to patients in their homes across Sarasota County, including North Sarasota, Longboat Key, University Parkway, and Venice. The role involves 5-10+ visits per week with possible full-time opportunities as the market grows.

Qualifications

• Active Florida PTA license is required.
• Minimum of 1 year experience as a PTA.
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills.

Responsibilities

• Provide physical therapy services to patients in their homes under the supervision of a Physical Therapist.
• Assist the Physical Therapist in evaluating equipment needs to maximize function.
• Educate patients and families on self-care and techniques to improve or maintain functioning.
• Collaborate with the interdisciplinary team to ensure holistic, seamless care.

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health PTA (Physical Therapy Assistant) to work with our Home Health team to deliver individualized care to patients in their homes in Sarasota county.

Locations: Home health visits in Sarasota county (including North Sarasota, Longboat Key, University Parkway, South Sarasota, Laurel, Osprey, Nokomis, and/or Venice)

Schedule : PRN role; 5-10+ home health visits per week. Full-time may be available as well!

What You’ll Do

• Provide physical therapy services to patients in compliance with agency policies and procedures, federal and state regulations, following the plan of care under the supervision of a Physical Therapist.
• Assist the Physical Therapist in evaluating equipment needs of patient to increase functional level.
• Participates in the education of patient and family on methods of self-care and techniques to improve or maintain patient’s level of functioning.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need
