Position: Pharmacist

Department: Pharmacy (IP)

Location: Sterling Hospitals, Sindhubhavan

Reporting To: Pharmacy In-charge / Unit Head

Employment Type: Full-Time

1. Job Purpose

To ensure safe, accurate, and timely dispensing of medicines to patients as per valid prescriptions and hospital protocols. The Pharmacist is responsible for medication management, inventory control, patient counselling, documentation, and compliance with applicable statutory and hospital standards.

2. Key Responsibilities

A. Prescription & Medication Management

  • Review prescriptions for completeness, dosage, frequency, route, and duration before dispensing.
  • Dispense medicines accurately as per the prescription and established pharmacy procedures.
  • Identify and elevate potential medication errors, drug interactions, contraindications, allergies, or unusual doses to the concerned physician/pharmacist in charge.
  • Ensure correct medicine, strength, dosage form, quantity, and labelling before dispensing.
  • Provide appropriate instructions to patients/attendants regarding medicine usage, storage, and precautions.
  • Maintain confidentiality of patient and medication‑related information.

B. Inpatient & Outpatient Pharmacy Operations

  • Process and dispense OPD and IPD medication orders within defined timelines.
  • Coordinate with nursing and clinical departments regarding medication requirements.
  • Ensure timely supply of medicines to wards, ICU, OT, emergency, and other clinical areas as applicable.
  • Handle emergency and stat medication requirements promptly.
  • Ensure proper documentation of issued and returned medicines.

C. Inventory Management

  • Monitor stock levels and ensure availability of essential and frequently used medicines.
  • Follow FEFO (First Expiry, First Out) principles for inventory management.
  • Regularly check medicines for expiry, damage, and storage conditions.
  • Segregate expired, damaged, recalled, or quarantined medicines appropriately.
  • Assist in stock verification and reconciliation.
  • Maintain appropriate stock levels and communicate shortages or near-stock-out situations to the Pharmacy In-charge.
  • Monitor and maintain records for high-value, high-alert, and controlled/scheduled medicines as applicable.

D. Storage & Medication Safety

  • Ensure medicines are stored under appropriate temperature, humidity, light, and other specified conditions.
  • Monitor and document refrigerator/cold-chain temperatures where applicable.
  • Ensure proper segregation and identification of look‑alike/sound‑alike (LASA) and high‑alert medications.
  • Follow medication safety protocols and hospital SOPs.
  • Report medication errors, near misses, adverse drug reactions, and other medication‑related incidents as per hospital policy.

E. Documentation & Compliance

  • Maintain accurate pharmacy records, registers, issue/return records, and other required documentation.
  • Ensure compliance with applicable pharmacy, drug‑control, narcotics/controlled‑drug, and hospital regulations.
  • Maintain records and documentation required for statutory inspections and hospital audits.
  • Support compliance with NABH and other applicable quality standards.
  • Participate in internal and external audits related to pharmacy operations.

F. Patient & Customer Service

  • Interact courteously and professionally with patients, attendants, doctors, nurses, and other hospital staff.
  • Provide appropriate medication‑related information within the scope of professional responsibilities.
  • Address routine pharmacy‑related queries and elevate clinical queries to the appropriate pharmacist/physician.
  • Ensure a patient‑centric approach while maintaining accuracy and medication safety.

G. Additional Responsibilities

  • Participate in pharmacy quality‑improvement initiatives and medication‑safety programs.
  • Attend departmental meetings, training programs, and competency assessments.
  • Maintain cleanliness, organization, and safety within the pharmacy area.
  • Follow hospital infection‑control, fire‑safety, and occupational‑safety guidelines.
  • Perform other duties assigned by the Pharmacy In-charge / Pharmacy Manager.

3. Educational Qualification

  • Diploma in Pharmacy (D.Pharm) / Bachelor of Pharmacy (B.Pharm) from a recognized institution.
  • Valid registration with the applicable Gujarat Pharmacy Council.
  • Additional qualifications in hospital pharmacy will be an advantage.

4. Experience

  • Relevant experience in a hospital pharmacy is preferred.
  • Candidates with experience in IPD/OPD pharmacy operations, medication inventory, and hospital pharmacy systems will be preferred.
  • Freshers may be considered based on organizational requirements.
