Job summary

SIMS Hospital in Chennai is seeking a dedicated nurse to join the dialysis unit to oversee patient care from admission to discharge. The role emphasizes adherence to policies, safety, infection control, and collaboration with the healthcare team.

The successful candidate will document nursing histories, assess patients, develop care plans, monitor vitals, educate patients and families, and participate in orientation and shift handovers, with dialysis-specific responsibilities as part of daily

Responsibilities

• Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
• Writes initial nursing histories, assessment & reassessment of patients conditions and develops individual care plans for patients assigned to the unit.
• Able to formulate Nursing Care plan
• Monitor patients vitals and report adverse reactions to medications in a timely manner
• Collaborate on discharge planning with the healthcare team
• Answer questions and educate patients and their families on surgical procedures and post-surgery care
• Informs nursing personnel of new admissions, their expected time of arrival, room assignment, etc.
• Implement and maintain established nursing objectives and standards.
• Notifies the Nurse Manager/ Charge Nurse when a patient is involved in an accident or incident.
• Participates in the orientation of new patient/family members to the dialysis unit
• Meets the need of the patients, and/or family members, as necessary. Report problem areas to the Nurse Manager/ Charge Nurse.
• Admits, transfers, and discharges patients as necessary according to Doctor’s order.
• Participates in the maintenance of cleanliness of treatment areas to ensure prevention of cross infection.
• Consults with the patient's attending physician in providing the patient's care, treatment, rehabilitation, etc., as necessary.
• Monitor and record the hemo dynamic parameters in the Nurses records as per policy
• Shall give priority for pain assessment and care as a vital sign when providing care to patients.
• Follows strict handing over and taking over policy.
• Ensures that the equipment and supplies including medications are ready for the following shifts.
• Make keen observation for early identification of warning signs of complication.
