Heritage Park Rehabilitation in Roy, UT is seeking a PRN Speech-Language Pathologist to join our in-house rehab program. You will screen and evaluate patients with communication, cognitive, or swallowing disorders and deliver rehabilitative treatment in line with regulatory and clinical standards.

The role requires a Speech-Language Pathologist license and is open to SLPs at all experience levels. You’ll coordinate with families and other providers and produce discharge summaries.

• License as Speech-Language Pathologist required.
• Open to SLPs at all experience levels.

Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.
• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.
• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to speech disorders.
• Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.

Heritage Park Rehabilitation in Roy, UT is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Speech-Language Pathologist for our in-house rehab program!

Key Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following regulatory and clinical standards.


• Deliver rehabilitative treatment to patients with communication, cognitive, and/or swallowing disorders, ensuring compliance with regulatory and clinical practice requirements.


• Evaluate treatment outcomes, modifying services based on patient evaluation and making referrals as needed.
