Job summary

Nextenti Tech is seeking a hospital clinical pharmacist to join our clinical team. The role focuses on reviewing prescriptions, ensuring safe medication use, and collaborating with physicians and nurses to optimize patient treatment plans.

The candidate will monitor therapy outcomes, provide drug information, and participate in rounds to support evidence-based care and antimicrobial stewardship programs.

Responsibilities

• Review and validate medication prescriptions for accuracy, dosage, drug interactions, and contraindications.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient treatment plans.
• Monitor patient medication therapy and evaluate treatment outcomes.
• Provide drug information and medication counseling to patients and their families.
• Identify, prevent, and resolve medication-related problems and adverse drug reactions.
• Participate in ward rounds and clinical discussions to support evidence-based patient care.
• Ensure safe and effective use of medications according to hospital protocols and clinical guidelines.
• Review patient medical histories, laboratory reports, and clinical data to recommend appropriate therapies.
• Educate healthcare staff on medication usage, safety practices and new drug therapies.
• Assist in developing and implementing medication management policies and procedures.
• Monitor antimicrobial usage and support antimicrobial stewardship programs.
• Maintain accurate documentation of clinical interventions and patient medication records.
• Participate in quality improvement initiatives and medication safety audits.
• Ensure compliance with regulatory, accreditation, and hospital standards.
