Remuneration will reflect the candidate’s experience and skills, subject to the approved budget
Established in 1993, YRGMERF is a pioneering non-governmental organisation in India, committed to transforming lives through integrated healthcare, inclusive partnerships, and forward-looking research. Grounded in the values of integrity and sustainability, we address both immediate and evolving societal challenges, empowering communities to attain equitable and lasting health outcomes. We provide comprehensive services across prevention, care, support, and treatment, particularly for those affected by HIV and other infectious diseases.
YRGMERF is anticipating a project to strengthen the Urban Ecosystem Against Future Disease Threats by implementing a framework for urban-peri-urban coordination. If commissioned, this project will require a team to support the coordination, implementation, and monitoring of several components.
The Patient Navigator will help clients registered at the Shimlapuri satellite centre stay on track with their routine tests, HIV care and TB screening. The role makes sure no client is lost to follow-up, including those who have moved out of the area, and that the centre runs smoothly on all working days. The Navigator works closely with the PSACS staff nurse, the Site Supervisor at Ludhiana ICC, the parent centre at Civil Hospital, Ludhiana, and linked NGOs and OST counsellors.
1. Routine testing: Follow up all clients every month for routine testing – VDRL, HIV, HCV and HBsAg – as per schedule, and keep test records up to date.
2. ART and lab monitoring: Follow up PLHIV clients for regular ART and adherence. Plan viral load and CD4 sampling as per schedule and make sure no due sample is missed.
3. TB screening and referral: Carry out 4S screening for TB among clients. Refer suspected cases to the parent centre at Civil Hospital, Ludhiana for further testing, and track each referral till the result is known.
4. Migrated-out clients: Follow up migrated-out clients by phone for testing and ART as per their schedule. Make home visits where needed, in coordination with linked NGOs and OST counsellors.
5. Centre management: Manage day-to-day running of the satellite centre when the main staff nurse (PSACS staff) is not present, and inform the Site Supervisor of any matter needing a clinical decision.
6. Reporting: Prepare the monthly report and submit it on time to YRG MERF and PSACS in the prescribed formats. Keep client registers, follow-up logs and referral records accurate and current.
7. Other duties: Take up any other related task assigned by the Site Supervisor in the interest of client care and programme delivery.
The Patient Navigator will report to the Site Supervisor, Ludhiana ICC, and will coordinate day-to-day work with the PSACS staff nurse at the Shimlapuri satellite centre.
