- Maintain and update patient medical records accurately and securely.
- Ensure proper filing, indexing, and retrieval of inpatient and outpatient files.
- Check completeness of patient records (discharge summaries, consent forms, reports, etc.).
- Assign and maintain Medical Record Numbers (MRN) for new patients.
- Handle requests for medical records from authorized personnel or departments.
- Maintain confidentiality and comply with data protection regulations.
- Prepare and submit monthly MRD reports to management.
- Assist in digitization/scanning of records and updating the hospital information system.
- Support internal and external audits by providing necessary documentation.
- Ensure safe archiving and disposal of old records as per policy.
Skills and Competencies:
- Good knowledge of medical record management procedures.
- Familiarity with hospital software systems (HIS/EMR).
- Attention to detail and accuracy in documentation.
- Strong organizational and communication skills.
- Understanding of confidentiality and legal aspects of medical records.
Qualification and Experience:
- Qualification: Diploma / Degree in Medical Record Technology or Health Information Management.
- Experience: 1-3 years of experience in a hospital MRD or related department.
- Preferred: Knowledge of NABH/JCI documentation standards.
