Anand Multispeciality in Anand, Gujarat, is seeking a capable Medical Records Assistant to coordinate discharge summaries, generate computerized reports, and manage documentation in EMR. The role requires 1-2 years of experience and regular(full-time) work in the hospital setting.

You will prepare certificates, ICD coding, and procedure notes, import data into Hinai, and maintain daily reports while liaising with consultants and resident doctors.

Responsabilidades

• Coordinate with medical officer for discharge summary and prepare computerized discharge summary from handwritten notes.
• Prepare final summaries with accuracy, add ICD coding and print two copies.
• Prepare certificates such as MLC Certificate, Rest Certificate, Fitness Certificate, Implant Certificate and fit to fly certificate as advised by consultants.
• Prepare all procedure reports including Cath Lab Procedures and Operative Notes (Minor and Major Operation).
• Import discharge summary in Hinai with medications.
• Prepare daily report and submit to reporting manager/ supervisor.

• Co-ordinatewith medical officer for discharge summary of patient and prepare thecomputerized discharge summary from the hand written summary prepared bymedical officer.
• PrepareFinal Summaries for the patient with accuracy, add ICD Coding and in-time andprint the two copies of the same.
• Preparethe certificates i.e MLC Certificate, Rest Certificate, Fitness Certificate,Implant Certificate and fit to fly certificate etc. as suggested by theconsultants.
• Prepareall procedure reports i.e Cath Lab Procedures and Operative Notes (Minor andMajor Operation).
• Importthe discharge summary in Hinai with medications.
• Preparedaily report and submit the same to reporting manager / immediate supervisor.
• Workas assigned by reporting manager / immediate supervisor for MT related job inany department.
• GoodCommunication with all employee, Resident Doctors and Hon. Consultant
• Identifymistakes in Summaries / Reports, and check with doctors to obtain the correctinformation.
• Provideinformation to supervisors and co-workers by telephone, in written form,e-mail, or in person.
• Enterall the history, investigation and prescribe medications in EMR (ElectronicMedical Record).
• Co-ordinatewith Echo Technician for 2D Echo Report of patient and prepare the computerizedReport from the Register maintained by Technician and upload the same data inHinai.
