South Mississippi Mental Health Center is seeking an I-CORT Community Support Specialist to provide intensive community-based services to adults with severe mental illness in home and community settings. The role focuses on teaching skills, coordinating with care teams, and promoting recovery through individualized plans.

The ICORT Community Support Specialist provides intensive community-based services to adults with severe and persistent mental illness who qualify for services under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
Provides direct one-on-one behavioral interventions to adults with severe and persistent mental illness in home and community settings to teach communication, social, coping, and daily living skills while reducing problematic behaviors. Provides services as specified on the weekly schedule. Team members have the ability to provide a variety of functions and must demonstrate a willingness to be flexible in their duties. Provides support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan. Provides intensive case management services with a lower staff-to-client ratio to allow for more frequent contact and support. Collaborates with family members, healthcare providers, and community resources to develop and implement comprehensive treatment plans. Serves as a liaison between community agencies, healthcare providers, and treatment teams to coordinate seamless care. Participates in outreach activities to promote adult mental health programs throughout the service area. Implements crisis prevention and intervention strategies for high-risk clients. Conducts regular home visits to monitor client stability and medication compliance. Facilitates skill-building groups and recovery-oriented activities. Provides transportation for clients to essential appointments when necessary. Assists clients in accessing community resources, including housing, employment, and benefits. Attends staffing on a regular basis, discusses appropriate cases with clinicians, and provides consultation/referrals on cases within the service area. Always maintains client confidentiality. Provides community support services in compliance with DMH standards. Documents appropriate activities in case note format that justify the need for treatment. Completes and submits service logs per agency policy. Maintains case record documentation that is adequate, appropriate, complete, and timely. Attends and participates appropriately in clinical staffing. Responds to clients' needs in an appropriate and timely manner. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment. Completes additional job duties assigned by supervisor. Participates in interagency collaboration for the provision of client services. Participates in 24-hour on-call rotation for crisis response for ICORT clients.
Bachelor's degree in behavioral science or related field. Eligible for Community Support Specialist certification through the Mississippi Department of Mental Health. DMH certification within 30 days of hire and maintained throughout duration of employment. Satisfactory completion of CPR prior to reporting to the work site. Satisfactory completion of CPI within 30 days of employment. Valid driver's license and auto liability insurance. Ability to transport clients in private vehicles when necessary.
Previous experience in a mental health setting strongly preferred. Previous experience working with adults with serious mental illness (SMI) in a treatment setting strongly preferred. Experience with community-based or mobile treatment programs preferred.
