Job Description

The DRM is responsible for managing and maintaining medical records and ensuring the accurate documentation, storage, retrieval, and confidentiality of patient information. The involves coordinating with clinical and administrative departments, organizing patient files, verifying medical documentation, and ensuring records are available when required. The DRM supports efficient medical record management and compliance with hospital documentation and information management policies.

• Manage the collection, organization, and maintenance of patient medical records.
• Ensure accurate filing and indexing of inpatient and outpatient records.
• Verify medical records for completeness and proper documentation.
• Coordinate with doctors and clinical departments to obtain missing medical information.
• Maintain patient records in physical and electronic formats.
• Ensure timely retrieval and delivery of medical records to authorized departments.
• Maintain confidentiality and security of patient health information.
• Monitor record movement and maintain issue-and-return registers.
• Organize discharge summaries, investigation reports, consent forms, and treatment records.
• Ensure medical records are properly labeled and stored.
• Support medical record audits and documentation reviews.
• Maintain records according to hospital retention and disposal policies.
• Assist authorized personnel with medical record requests.
• Coordinate with billing, insurance, and TPA departments for required documentation.
• Ensure compliance with hospital policies and applicable medical record regulations.
• Identify incomplete or misplaced records and take corrective action.
• Maintain records of medico-legal cases as per hospital protocols.
• Prepare periodic reports related to medical record activities.
• Support digitization and scanning of patient records.
• Ensure proper use of medical record management systems.
• Report documentation issues and operational concerns to the concerned authority.
