Qualifications

Responsibilities

• Coordinate with the medical team for necessary updates for the patients.

• Prepare weekly, monthly and quarterly reports and data.

ROLEDESCRIPTION:

The Data Manager is responsible to ensure smooth transition through thecontinuum of care to achieve clinical, financial and satisfaction outcomesfor case management. Provides resources for allhospital and clinic patients as needed. The primary responsibilities surroundpsychosocial services and discharge planning/transition management functionsfor assigned patient population. Coordinate with medical teamfornecessary updates for the patients and help bridge/ facilitate a dialoguebetween patients and medical team

KEY RESULTAREAS:

DUTIESAND RESPONSIBILITIES:

Operational:

• Responsible for handling all the work assignedfor the Unit that includes:
  • (a) Going to Hospital Unit to assist in running of Chattai clinic and coordinate andimplement Cankids support activities as determined for the Center.
  • (b) Coordinatewith medical teamfor necessary updates for the patients and helpbridge/ facilitate a dialogue between patients and medical team
  • (c) Enteringdetails of child patient into database (Salesforce/vCan) from P.I.S. andM.S.F and complete all related procedures for the same.
  • (d) Form abridge between the doctors and the patient family and make sure the child isgiven the best care and benefit and spreading the message ‘’you are not Alone‘’ amongst the families and in the society.
  • (e) Updatethe database as when a child is adopted or money is donated for histreatment.
  • (f) Timely& periodically follow up with patients for continuing treatment.

DataManagement and Reports:

• (a) Maintainingthe data of the particular unit assigned.
• (b) Assist in making weekly/ monthly/ quarterlyreports in regular contact with Program Officer (Medical and HospitalPrograms).

Administrative:

• (a) Ensure proper assistance to pediatricCancer unit.
• (b) Spreading awareness about childhood cancerthrough awareness and information program.
• (c) Responsible for handling the unit asassigned
• (d) Responsible for maintaining propercoordination with the Medical Officer.
