Job summary

Zeraphos Care in Thiruvalla, Kerala, is seeking a Care Manager to oversee day-to-day care experiences for a portfolio of elders. You will coordinate home visits with CareMates, liaise with families, and ensure timely appointments with clinics and labs while keeping elders seen and heard.

Qualifications

• A background in care: healthcare, home care, eldercare, or nursing.
• Strong judgment and the ability to recognize when to escape a concern.
• Excellent verbal and written communication skills.
• Fluent in English and Malayalam.
• Comfortable using WhatsApp, email, and standard office software, and able to pick up new tools quickly.
• Experience working directly with families is preferred.

Responsibilities

• Appointments and follow-ups: ensuring accompaniment, lab follow-ups, and doctor appointments occur and communicating outcomes to families.
• Continuous improvement: helping raise the standard of care week over week.

The role

As a Care Manager, you'll own the day-to-day care experience for a group of elders: coordinating home visits, supporting our field care professionals (CareMates), speaking with families, and making sure every elder in our care feels seen, heard, and looked after.

This is an on-site role based in Thiruvalla. Some days you will be out visiting family homes and meeting CareMates in the surrounding area; others you will be coordinating from base, working with the clinics and labs our families already use.

Who we're looking for
