Job summary

ALONE is seeking a dedicated professional in Ireland to coordinate services for older people, advocate for their needs, and manage referrals across health and social care networks. You will develop personalised plans, monitor KPIs, and ensure accurate CRM data management.

You will build partnerships with statutory and non-statutory agencies, maintain referral pathways, and report on service developments while coordinating technology installations for safety and wellbeing.

Qualifications

• Evidence of ability to plan personalised support for older people.
• Strong problem-solving skills for needs assessment and resource allocation.
• Excellent written and verbal communication abilities.
• Ability to foster partnerships and negotiate with diverse stakeholders.
• Proficiency in technology and data management systems.

Responsibilities

• Devise personalised support plans to address challenges and enhance older people’s quality of life.
• Provide access to ALONE’s services and coordinate access to other community services.
• Advocate for older people’s needs within health, social care, and welfare services.
• Establish and maintain referral relationships with statutory and non-statutory agencies.
• Develop and maintain referral pathways for ALONE’s Support Coordination services.
• Adhere to ALONE’s quality management systems and policies.
• Monitor and manage KPIs and SMART targets.
• Deliver timely reports on service developments, challenges, and achievements.
• Manage distribution and installation of technology supporting older people’s wellbeing and safety.
• Maintain records in ALONE’s CRM database.
