Job summary

ALONE is seeking a dedicated Support Coordinator in the South Tipp/North Waterford region to assess needs, coordinate services, and connect older individuals with community supports. The role is field-based and requires travel within the community, a full driving licence, and own transport.

You will build personalised support plans, advocate for clients across health and welfare services, and maintain accurate CRM records while working with partners to optimise service delivery.

Responsibilities

• Provide support coordination by devising personalised support plans to address challenges and find solutions to enhance the older persons quality of life.
• Provide access to ALONE’s services while coordinating and enabling older people to access other services in the community.
• Advocate for older people's needs within health, social care and welfare services, working to streamline access and enhance service delivery.
• Establish and nurture strong referral relationships with both statutory and non-statutory agencies in the designated area, fostering collaboration and access to a wider range of support services.
• Develop and maintain efficient referral pathways for ALONE’s Support Coordination services, optimising service coordination.
• Adherence to ALONE's Quality management systems and policies, ensuring a consistent and high-quality service delivery approach.
• Monitor and manage key performance indicators (KPIs), striving to achieve SMART targets that maximize the reach and impact of support services for older people.
• Deliver thorough and timely reports on service developments, challenges, and achievements.
• Manage the distribution and installation of technology that enhances older people's well-being and safety.
• Maintain records on ALONE’s CRM database, capturing essential details to inform service delivery and future strategies.
