The role focuses on person-centred support for adults with intellectual disabilities and autism, helping individuals set and pursue personal goals, while promoting independence and community participation. You will work with families and professionals to implement person-centred plans, ensuring quality care aligns with policies and values.

Qualifications

• A relevant third-level qualification in Social Care.
• CORU registration as a Social Care Worker.
• Experience supporting adults with intellectual disabilities and/or autism.
• Strong communication, organisational and teamwork skills.
• A full, clean manual driving licence.

Responsibilities

• Provide person-centred support to adults with intellectual disabilities and autism.
• Support individuals to identify and work towards their personal goals.
• Promote independence, choice and inclusion.
• Support participation in social, recreational and community activities.
• Develop and maintain person-centred plans.
• Work collaboratively with families, colleagues and other professionals.
• Support delivery of services in line with New Directions and Prosper Meath's policies and values.
