Taconic Rehabilitation & Nursing at Ulster is seeking an RN Unit Manager to lead a nursing unit within Highland, NY. You will supervise clinical care, ensure regulatory compliance, and coordinate with interdisciplinary teams to deliver resident-centered care.

Qualifications

• Associate Degree in Nursing required; Bachelor’s degree preferred.
• NYS RN license required.
• Minimum 1 year RN experience in long-term care.
• 3–5 years long-term care experience with supervisory experience preferred.

Responsibilities

• Oversee the clinical care and daily operations of an assigned nursing unit.
• Supervise nursing staff, nursing assistants, rehabilitation aides, and unit support staff.
• Conduct resident assessments and ensure care plans are implemented, updated, and followed appropriately.
• Monitor resident conditions and communicate changes with providers, families, and interdisciplinary team members.
• Review provider orders, laboratory results, incident reports, and quality indicators.
• Participate in care plan meetings, family meetings, QAPI activities, and interdisciplinary rounds.
• Coordinate admissions, discharges, transfers, outside appointments, and resident services.
• Ensure accurate documentation, medication management, and regulatory compliance.
• Support staff education, orientation, coaching, and performance management activities.
• Monitor infection control, immunization tracking, resident safety, and quality improvement initiatives.
• Address resident, family, and staff concerns while promoting a positive care environment.

Requirements

• Associate Degree in Nursing required; Bachelor's Degree preferred
• Valid NYS Registered Nurse (RN) License required
• Minimum 1 year of RN experience in a long-term care setting required
• 3-5 years of long-term care experience with supervisory experience preferred
• Strong knowledge of resident assessments, care planning, and long-term care regulations
• Leadership experience with the ability to supervise, coach, and mentor staff
• Excellent communication, organizational, and problem-solving skills
• Ability to coordinate care across multiple disciplines and manage competing priorities
• Experience with quality improvement, infection prevention, and regulatory compliance preferred
• Commitment to providing compassionate, resident-centered care
