Job summary

Connected Health Group Limited is seeking a Rapid Response Healthcare Assistant to deliver urgent, hands-on care in clients’ homes across Ireland. You’ll support immediate needs, join new care packages, and help keep people safe, comfortable and independent in their communities.

This role requires a driving licence and car, at least 1 year in homecare, QQI Level 5 in Health & Social Care, and the ability to stay calm under pressure while communicating clearly and organizing visits and reporting.

Qualifications

• 1+ year experience in homecare in Ireland.
• QQI Level 5 in Health & Social Care is required.
• Driving licence and own car are essential for travel between clients.
• Must remain calm under pressure and communicate clearly.
• Strong organisation and compassionate care are essential.
• Weekend flexibility is required for rota-based shifts.

Responsibilities

• Shadow staff to ensure Health & Safety policies and procedures are followed.
• Undertake rapid response tasks including filling new packages and discharges.
• Monitor and report on quality of care to the area manager.
• Collect journal notes from clients' homes and ensure documentation is up to date.
• Deliver care plans, log sheets, gloves, and PPE to clients’ homes as needed.
• Undertake any other reasonable duties as required.
