Job summary

Connected Health Group Limited is seeking a Rapid Response Healthcare Assistant to deliver urgent, hands-on care to people in their own homes. You will support service users who need immediate assistance, step into new care packages, and help ensure individuals remain safe, comfortable, and independent in their communities.

This is a dynamic role for carers who are flexible, proactive, and passionate about high-quality care when it matters most.

Qualifications

• Experience in homecare (1+ year in Ireland).
• QQI Level 5 in Health & Social Care is required.
• Driving licence and access to a car for travel between clients.
• Calm under pressure with strong communication & organisation.

Responsibilities

• Staff shadowing to ensure Health & Safety policies and procedures.
• Rapid response: manage new care packages and discharges.
• Evaluate and monitor the quality of care delivered to clients and report concerns.
• Delivery of Care plans and journals to clients' homes when required.
• Delivery of PPE to client homes when required.

• QQI Level 5 in Health & Social Care - a formal qualification is required


• Driving licence + car - essential due to travel between clients

Key Duties And Responsibilities

• To be responsible for staff shadowing - ensuring Health & Safety policies and procedures


• To undertake emergency over - Rapid response (filling new packages and discharges)


• To evaluate and monitor the quality of the care delivered to clients and report to the area manager with concerns


• Checking and collecting journal notes from clients houses


• Delivery of Careplans, log sheets and gloves to client's homes when required


• Delivery of PPE

To undertake any other reasonable duties as required*
