Qualifications

Responsibilities

• Conduct regularly scheduled consumer visits in homes and community settings.
• Provide rehabilitation and support services per the Individual Rehabilitation Plan (IRP).
• Lead rehabilitation groups and activities addressing client needs.
• Maintain consistent documentation including notes, summaries, and reviews.
• Participate in ongoing training and supervision.
• Represent the organization to clients and families while upholding policy and standards.
