Job summary

Sunrise Group is seeking a Direct Support Professional PRN to provide support, supervision and assistance to individuals with intellectual and developmental disabilities in community settings.

Responsibilities include implementing person-centered plans, monitoring health and safety, and reporting any concerns. The role requires a background check, a valid driver’s license, basic computer skills and relevant experience as indicated by location.

Qualifications

• Valid Driver's License required.
• Basic computer skills required.
• Minimum one year of experience in the human service field where indicated by location.
• High School Diploma or equivalent required by work location.

Responsibilities

• Identifies and reports any suspected abuse or violations of an individual’s rights.
• Provides support, assistance and supervision to meet the needs and goals of the individuals according to their plans.
• Ensures that each individual’s training, behavior and support programs are documented in accordance with regulatory requirements.
• Ensures health, safety and well-being of individuals at all times.
• Provides a positive environment that promotes independence and choice.
• Follows positioning schedules and uses equipment per plan.
• Assists with diet, texture and equipment needs.
• Keeps homes, outside areas and vehicles well maintained and organized.
• Performs all other duties as assigned.
