Job summary

The Rehab Group is seeking a Programme Facilitator for the Hayfield Resource Centre in Thurles, Co. Tipperary. This permanent, 30-hour role focuses on promoting independent living and community integration through person-centred programmes and activities.

You will deliver tailored activities, work as a key worker, and maintain progress records while adhering to Rehab Group policies. The position offers extensive staff benefits and development opportunities.

Qualifications

• Minimum QQI Level 5 in Social Care or equivalent.
• Good knowledge of New Directions and community-based programmes.
• Knowledge and understanding of intellectual disabilities and mental health issues.
• 1 year minimum experience in a relevant setting with experience of facilitating and implementing person-centred plans.

Responsibilities

• Develop and deliver person-centred programmes to meet the needs of service users and maintain required records.
• Be a key worker to service users to facilitate the Person Centred Planning process; identify goals and supports and maintain records.
• Support individuals and groups in accessing their community by facilitating activities and programmes and other duties as deemed appropriate by the supervisor.
• Observe all Rehab Group rules, regulations, policies and procedures at all times.

Job Purpose

To promote and support independent living and community integration. To facilitate in the educational, developmental, recreational and occupational programmes and activities of the service, as appropriate. Programmes will be designed to match the service user’s needs and capabilities while allowing him/her to develop his/her full potential and will be implemented at an appropriate time and setting in order to give maximum support.

What we’re looking for:

Your Responsibilities

• To develop and deliver person centred programmes to meet the needs of service users as identified through Person Centred Planning and to maintain the required records
• To be a key worker to service users in order to facilitate the Person Centred Planning process. To identify goals and supports required by individual service users and maintain records as required.
• To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.
• To observe all rules and regulations of the RehabCare services ensuring that all relevant Rehab Group administrative rules, regulations policies and procedures are adhered to at all times

Salary starting from point 1 € 33,217.35 to point 13 € 35,924.82 (pro rata) and issubject to negotiation depending on experience and qualification.
