The Rehab Group in Kerry is seeking a Programme Facilitator to support service users in choosing and developing person-centred programmes, including independent living and community activities, within a quality of life framework.

You will work under the Community Service Manager, with qualifications in Social Care and experience with intellectual disability. A full, clean driving licence is desirable; police certificates may be required.

Competenze

Experience implementing person-centred plans for adults with intellectual disability.
Experience facilitating programmes and activities to meet service users' needs.
Ability to support independent living and community integration.

Programmes Facilitator - various roles open throughout Kerry

About the Role

Reporting to the Community Service Manager through the Programmes Supervisor, the post holder will facilitate service users in their choices, developing programmes within the context of a person centred model of service delivery. The programmes may include independent living, personal development and occupational and leisure activities, within the context of a quality of life model.

What We’re Looking For

Minimum QQI Level 5 in Social Care or equivalent
A third level graduate, Health & Social Care or equivalent is desirable.
Experience supporting adults with an intellectual disability and people enduring mental health issues
1 year minimum experience of working in a relevant Settingwith the experience of facilitating and implementing personcentredplans, experience in the design, development and provision ofprogrammesand activities to meet the needs, choices and abilities of service users including providing support in the area of independent living skills and community integration for persons with intellectual disability.

Your Responsibilities

To be a key worker to service users in order to facilitate the Person Centred Planning process. To identify goals and supports required by individual service users and maintain records as required.
To develop and deliver person centred programmes to meet the needs of service users as identified through Person Centred Planning and to maintain the required records.
To support individuals and groups in accessing their community to meet identified needs by facilitating activities and programmes and any other reasonable duties as deemed appropriate by the Programmes Supervisor / Community Services Manager.

Additional Requirements

A full, clean driving licence, with a minimum of 2 years’ experience is desirable.

If applicable, candidates will need to provide a police certificate for any country they've lived in before moving to Ireland

A panel may be formed as a result of this campaign.

We offer a competitive starting salary of € 33,217.35–€ 34,939.17 per annum (Points 1–5 of the scale), based on qualifications and experience. The 12-point salary scale provides annual progression opportunities up to€ 35,924.82 per annum.
