Job summary

The Rehab Group in Galway is seeking a Programme Facilitator for a 12-month fixed-term contract of 15 hours per week. The role reports to the Community Service Manager and involves running person-centred programmes for service users, including independent living and personal development.

Requirements include QQI Level 5 in Social Care, experience with adults with disabilities or mental health issues, and the ability to work as a lone worker with travel.

Qualifications

• Minimum QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people with mental health issues.
• Programme facilitation skills and client handling experience are advantageous.
• Experience of lone working and community-based support.

Responsibilities

• Be a key worker for service users, enabling Person-Centred Planning and maintaining records.
• Develop and deliver person-centred programmes and maintain required records.
• Support individuals and groups to access community activities and services.
• Work as a lone worker and liaise with team to ensure high-quality service.
• Communicate with external agencies and families and keep relevant records.
• Follow Rehab Group policies and ensure safety, hygiene and housekeeping guidance.
• Promote independence and community integration in collaboration with community personnel.
