Job summary

Care Connect Group IRL is seeking compassionate Community Support Workers to help people live independently in their own communities. You will provide person-centred support in clients’ homes and local settings, supporting daily living activities and promoting dignity, choice and independence.

Join a multidisciplinary team, build meaningful relationships, and assist with appointments, community activities and everyday routines.

Qualifications

• Minimum QQI Level 5 qualification in Healthcare Support / Social Care / Community Care / Applied Social Studies.
• Experience with disabilities or elderly care is beneficial.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Good organisational and record-keeping skills.

Responsibilities

• Provide compassionate, person-centred support in homes and communities.
• Support activities of daily living and personal routines with dignity.
• Promote independence by encouraging self-reliance and choice.
• Help individuals access local community activities and events.
• Accompany to appointments, shopping, social activities and hobbies.
• Develop meaningful relationships and social connections.
• Encourage participation in education, employment and other activities.
• Maintain safe, welcoming home environments and follow care plans.
• Document interactions and communicate with families and professionals.
