Job summary

Medforce is recruiting a Social Care Worker to join a Primary Care Social Work Team in Dublin. You will provide person-centred support to individuals in the community, promoting independence and wellbeing within a multidisciplinary setting.

The role emphasizes care planning, advocacy and collaboration with families, healthcare and community partners to achieve best outcomes for service users.

Qualifications

• Strong knowledge of social care and social work practice.
• Ability to apply interventions meeting individual needs.
• Excellent assessment, decision-making and problem-solving skills.
• Ability to manage a busy caseload and prioritise tasks.
• Excellent communication and interpersonal skills.
• Commitment to person-centred, user-focused services.
• Proficiency with Word, Excel and email systems.

Responsibilities

• Provide high-quality social care support per standards and policies.
• Build positive relationships with individuals and families.
• Promote rights, dignity, independence and wellbeing.
• Support care planning and participate in MDT decisions.
• Implement and review care plans with the team.
• Assist with home management, budgeting and personal hygiene.
• Organise documentation and finances; attend medical appointments.
• Support access to education, training and community activities.
• Act as a keyworker when required; coordinate supports.
• Liaise with agencies to ensure appropriate services.
