Job summary

Medforce Healthcare Recruitment is seeking a Professionally Qualified Social Worker (P.Q.S.W.) to join a Primary Care service in South Dublin. The role focuses on delivering person-centred social work interventions within a multidisciplinary community-based team.

Requirements include a recognised Social Work qualification and CORU registration. Excellent communication and caseload management skills are essential for success in this role.

Qualifications

• Recognised qualification in Social Work.
• CORU registration as a Social Worker is essential.
• Experience in community health or Primary Care settings is desirable.
• Strong knowledge of professional practice and relevant legislation.
• Excellent communication, interpersonal and organisational skills.
• Ability to manage a caseload effectively and work independently.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Provide professional Social Work assessment and intervention to service users and families.
• Manage an allocated caseload in line with professional standards and service requirements.
• Identify and assess social care needs and develop intervention plans.
• Collaborate with service users, families, carers and other professionals.
• Liaise with multidisciplinary teams and external agencies as appropriate.
• Advocate for service users and support access to community and social care services.
• Maintain accurate case records and documentation.
• Participate in multidisciplinary meetings and contribute to care planning.
• Work in accordance with legislation, policies and best practice.
