Job summary

Medforce is recruiting a Social Care Worker to join a Primary Care Social Work Team in Dublin. You will provide high-quality, person-centred social care support within a multidisciplinary team, helping individuals achieve greater independence and wellbeing.

You will work with care plans, promote dignity, and liaise with healthcare and community professionals to coordinate services and outcomes for service users.

Qualifications

Responsibilities

  • Provide high-quality social care support in accordance with professional standards, legislation and relevant policies.
  • Develop positive, respectful and professional relationships with individuals and their families.
  • Promote the rights, dignity, independence and wellbeing of service users.
  • Support individuals to participate in care planning and decision-making.
  • Implement and review care plans and interventions developed by the Social Work Team.
  • Support individuals with home management, including budgeting, cleaning and personal hygiene.
  • Assist with the organisation and management of personal documentation, information and finances.
  • Accompany and support individuals attending medical, clinical and community appointments.
  • Support individuals to access education, training, social and community activities.
  • Act as a keyworker where required.
  • Coordinate and maintain networks of formal and informal supports around the individual.
  • Liaise with statutory and voluntary agencies to facilitate appropriate services and supports.
  • Participate in case reviews, meetings and multidisciplinary team working.
  • Provide clear written and verbal updates to the Social Work Team and wider MDT.
  • Promote safeguarding and ensure individuals are protected from abuse, bullying and other risks.
  • Participate in crisis management in accordance with relevant policies and procedures.

The Role

Medforce is currently recruiting a Social Care Worker to join a Primary Care Social Work Team based in Dublin.

This is an excellent opportunity for a dedicated Social Care Worker to make a meaningful difference in the lives of individuals living in the community. The successful candidate will provide high-quality, person-centred support, helping individuals to achieve greater independence, wellbeing and quality of life.

Working as part of a wider multidisciplinary team (MDT), you will support individuals in line with their individual needs, care plans and personal goals, promoting dignity, choice, independence and participation in everyday life.

Key Responsibilities
  • Provide high-quality social care support in accordance with professional standards, legislation and relevant policies.
