Job summary

Medforce Recruitment is seeking a Social Worker to join the Dublin South West Integrated Care Programme for Older Persons (ICPOP) Community Specialist Team. You will provide specialist social work assessment, case management and support to older people and families across community, primary care and hospital settings.

This role requires collaboration with a multidisciplinary team, a commitment to person-centred care, advocacy, and ensuring safe transitions between care settings.

Qualifications

• Hold a recognised qualification in Social Work.
• Be registered, or eligible for registration, with CORU.
• Provide evidence of statutory registration before a contract is issued.
• Have experience working with older people and complex care needs.
• Demonstrate knowledge of the Assisted Decision-Making (Capacity) Act.
• Know the Decision Support Service (DSS) and DMRR processes.
• Familiarity with the Nursing Home Support Scheme (Fair Deal).
• Excellent communication and interpersonal skills.
• Strong assessment, advocacy and case-management abilities.
• Full driving licence and access to own transport.
• Willing to complete Garda Vetting.

Responsibilities

• Manage a complex caseload of older people within the community.
• Conduct comprehensive social, psychological and practical assessments.
• Contribute to geriatric assessment and multidisciplinary care planning.
• Develop person-centred care plans with older people, families and carers.
• Identify appropriate social and community supports.
• Support safe, preferred home living whenever possible.
• Advocate for the rights and preferences of older people.
• Liaise with primary care, community and acute hospital services.
• Participate in supervision, team meetings and continuing professional development.
• Contribute to service evaluation, audit and quality improvement.
