Job summary

CRC Central Remedial Clinic is seeking a Social Care Worker for its Adult Services in Clontarf. The role focuses on delivering person-centred supports that promote independence, inclusion and meaningful life outcomes, with responsibilities as a key worker and lead in individual support planning.

You'll work with adults to access education, employment and community activities, while upholding safeguarding standards and collaborating with families, advocates and colleagues across the service.

Qualifications

• Degree in Social Care Practice (or equivalent qualification eligible for CORU registration).
• Minimum one year's experience supporting adults with disabilities, autism and/or complex support needs.
• Experience facilitating person-centred planning and outcome-focused reviews.
• Knowledge of New Directions and community inclusion principles.
• Full clean manual Irish driving licence.

Responsibilities

• Act as Key Worker for designated individuals, coordinating supports and person-centred planning processes.
• Develop, implement and review individual support plans focused on meaningful outcomes.
• Support adults to access education, employment, volunteering, recreation and community opportunities.
• Promote rights-based practice and supported decision-making in line with the Assisted Decision-Making (Capacity) Act.
• Deliver personal care, medication supports and positive behaviour support interventions as required.
• Work collaboratively with multidisciplinary colleagues, families, advocates and community partners.
• Support the day-to-day operation of the service and provide guidance to Community Support Workers.
• Maintain high standards of safeguarding, governance, record-keeping and professional practice.
