Job summary

Medforce in Athlone seeks a Social Worker for the CDNT to provide professional social work services to children with disabilities (0-18) and their families, promoting wellbeing and inclusion.

You will conduct psychosocial assessments, develop care plans with the MDT, and advocate for access to services while ensuring GDPR/CORU compliance and safeguarding. A recognised qualification and CORU registration are required.

Qualifications

• Recognized Social Work qualification.
• Registration with CORU as a Social Worker.
• Post-qualification experience with children and families; disability services experience an advantage.
• Ability to work independently and as part of a team.
• Full driving licence and access to a vehicle for home visits.

Responsibilities

• Conduct psychosocial assessments of children with disabilities and their family contexts.
• Develop, implement, monitor and review individualized social work support plans with the child, family and multidisciplinary team.
• Provide direct social work interventions: counselling, crisis intervention, safeguarding.
• Liaise with health, education, community and voluntary services to access resources.
• Advocate for children’s rights and entitlements and ensure access to services.
