Job summary

Connected Health Group Limited is seeking a dedicated Care Co-ordinator to work with the Client Care Manager to monitor and develop services across the CHO geography.

You will oversee rosters and timesheets, analyse opportunities for growth, prepare weekly and monthly reports, and liaise with HSE, service users, carers and other health professionals.

The role includes providing excellent customer service, coordinating referrals, and supporting the day-to-day management of client care operations.

Responsibilities

• Oversee rosters and timesheets, ensuring accurate scheduling and coverage.
• Analyse opportunities for growth and communicate staffing needs to recruitment.
• Prepare weekly and monthly management reports on efficiency and quality of runs.
• Liaise with HSE, prospective service users, NOK and health professionals to coordinate care.
• Respond to referrals and coordinate new packages from HSE and private sector.
• Maintain high level of customer service in interactions with clients, carers and CCM.
