Job summary

Hollybank Trustees Ltd. is seeking a Care Co-ordinator to support the Client Care Manager, monitor service delivery, and liaise with health professionals and external agencies in Dundalk.

The role requires coordinating rosters, managing referrals, and maintaining quality across the service. You will work standard business hours and play a key role in expanding service coverage while delivering high levels of customer service to clients and carers.

Qualifications

• Effective leadership and management in growing business hours.
• Efficient use of IT systems.
• High level of customer service to calmly deescalate complaints and concerns.
• Successful relationship building and representing a business externally.

Responsibilities

• Assist and work closely with the Client Care Manager to monitor and grow area within the company.
• Liaise with Care Managers, HSE, prospective Service Users, NOK, health professionals and other agencies connected with training and development of the service.
• Liaise with Care Assistants on daily queries.
• Monitor client care calls including missed and late calls.
• Prepare weekly and monthly reports showing efficiency and quality.
• Maintain accurate logs of client and carer concerns and ensure proper escalation.
