Job summary

Siloam Hospitals Simatupang in Jakarta/South Jakarta region seeks a Clinical Pharmacist to provide comprehensive clinical pharmacy services within our hospital setting.

You will review prescriptions, monitor drug therapy, counsel patients, and work with physicians and nurses to optimize patient outcomes and safety. Hospital experience and valid STR/SIP are preferred.

Qualifications

• Master’s Degree (S2) in Clinical Pharmacy from a recognized university.
• Possess a valid Pharmacist Registration Certificate (STR).
• Possess a valid Pharmacist Practice License (SIP) or meet the requirements to obtain one.
• Previous experience in hospital clinical pharmacy is preferred.
• Strong knowledge of clinical pharmacy practice, pharmacotherapy, medication safety, and patient care.
• Strong analytical, communication, and interpersonal skills.
• Able to collaborate effectively with physicians, nurses, and other healthcare professionals.
• Detail-oriented, proactive, responsible, and patient oriented.

Responsibilities

• Provide clinical pharmacy services in accordance with professional standards and hospital policies.
• Review prescriptions and medication orders to ensure appropriateness of drug selection, dosage, route, frequency, and potential drug interactions.
• Conduct medication reconciliation and medication reviews as required.
• Monitor effectiveness and safety of drug therapy and identify Drug-Related Problems (DRPs).
• Provide evidence-based recommendations regarding drug therapy to physicians and other healthcare professionals.
• Monitor and report adverse drug reactions (ADRs), medication errors, and other medication-related problems.
• Provide medication counseling and education to patients and their families.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes and ensure medication safety.
• Ensure accurate and timely documentation of clinical pharmacy activities.
• Participate in quality improvement and patient safety programs related to medication management.
• Perform other duties related to clinical pharmacy services as assigned by the supervisor.
