Job summary

AWWA LTD in Hong Kong is seeking a Social Worker to ensure psycho-social and emotional well-being of residents and to provide person-centred care under the Social Work Code of Ethics. You will collaborate with interdisciplinary teams to formulate ICPs and manage case coordination, discharge planning and monthly reporting.

The role requires 2–4 years of social work experience in community care or residential settings, with fluency in English and a working knowledge of PDPA and SASW standards.

Qualifications

• Degree or Graduate Diploma in Social Work.
• Registered or eligible for SASW registration.
• Knowledge of Social Work Code of Ethics, case management frameworks and PDPA.
• 2–4 years of relevant social work experience preferred.
• Understanding of geriatric care landscape in Singapore/HK context.

Responsibilities

• Provide psycho-social and emotional well-being support to residents.
• Coordinate services, monitor well-being and discharge planning.
• Develop ICPs based on BPSS model and holistic care philosophy.
• Plan and conduct group work to build coping abilities.
• Maintain confidential, timely documentation and reporting.
