You will work with caregivers and families to age-in-place safely, and collaborate with multidisciplinary teams. The role requires 5–8 years of relevant experience in healthcare or eldercare social work, SASW recognition, and supervisory

Qualifications

• Degree or Postgraduate qualification in Social Work recognised by SASW.
• Registered Social Worker (RSW) preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments for clients and caregivers and develop care plans.
• Provide counselling, emotional support and crisis intervention to clients and families.
• Work with nurses, doctors and therapists in multidisciplinary case conferences and care planning.
• Provide guidance and clinical supervision to junior social workers and interns.
• Maintain accurate case documentation and reports in line with policies.
• Participate in quality improvement, service development and audits.
