Job summary

NTUC Health Co-operative Limited is hiring Social Workers across its Active Ageing Centre, Senior Day Care and Home Care services. The Social Worker conducts psychosocial assessments, counselling, caregiver support, and care coordination to promote wellbeing and independence.

Guided by Journey-Based Care and person-centred practice, you will work with seniors, caregivers, and multidisciplinary teams to develop integrated care plans, facilitate referrals, and support ageing in place across

Qualifications

• Degree or Postgraduate Degree in Social Work recognised by SASW.
• Registered Social Worker (RSW) preferred or eligible for registration.
• Minimum 2-5 years of experience in community care, eldercare, medical social work, home care or social service settings.
• Experience working with frail older adults and caregivers is preferred.
• Knowledge of Singapore's community care ecosystem and financial assistance schemes.
• Experience in multidisciplinary care coordination is an advantage.

Responsibilities

• Champion Journey-Based Care and Esther Care Philosophy in daily practice.
• Engage seniors and caregivers with 'What Matters Most' to understand goals and priorities.
• Conduct psychosocial, caregiver, functional and financial assessments to identify needs and resources.
• Develop, implement and review integrated One Care Plans with seniors and caregivers.
• Provide psychosocial assessment, counselling and crisis intervention.
• Support seniors and caregivers facing adjustment difficulties, grief, isolation, caregiver stress and dementia.
• Coordinate referrals, linkages to internal and external services, and mobilise resources.
• Coordinate care across multidisciplinary teams and community partners; participate in care conferences.
• Maintain timely, accurate documentation and contribute to quality improvement initiatives.

Education

Degree or Postgraduate Degree in Social Work recognised by SASW

Registered Social Worker (RSW) preferred
