Job summary

Hong Kong Red Cross invites applications for a contracted community health support role in West Kowloon.

The successful candidate will assist in planning and coordinating elder-care services, including the Home-based Chronic Disease Management Service for the Elderly, with a focus on vulnerable and multicultural communities.

Qualifications

  • 5 subjects passed in HKCEE/HKDSE (including English and Chinese) or University Degree holder.
  • Self-motivated, responsible, flexible and able to work independently and in a multi-disciplinary team under pressure.
  • Experience in volunteer coordination, programme planning and implementation, or elderly service is preferred.
  • Good communication and interpersonal skills, with willingness to work with people from diverse backgrounds.
  • Proficient in MS Word, Excel, PowerPoint and Chinese word processing.

Responsibilities

  • Support the Assistant Manager in planning, implementing and coordinating community health services, including the Home-based Chronic Disease Management Service for the Elderly (CDME) and related initiatives for vulnerable and multicultural communities, with a focus on Tai Po CDME cases.
  • Organize and deliver services, training and activities to enhance chronic disease self-management, home safety awareness, social support and community connection for the target population.
  • Provide administrative, logistical and inventory support to ensure smooth and efficient operation of community health services, including preparation of programme materials, service records and activity-related documents.
  • Compile and update service statistics, case records and programme data, and coordinate with volunteers, service providers and stakeholders to support service evaluation, reporting, audit preparation and continuous improvement.
  • Liaise with volunteers, service providers, community partners and internal teams to facilitate service delivery and follow-up actions.
  • Carry out ad hoc duties and projects as assigned by the supervisor.

Key Responsibilities

Requirements
  • 5 subjects passed in HKCEE/HKDSE (including English and Chinese) or equivalent; or University Degree holder in any discipline.
  • Self-motivated, strong sense of responsibility, flexible and able to work both independently and in a multi-disciplinary team under pressure.
  • Experience in volunteer coordination, programme planning and implementation, community service implementation or elderly service is preferred.
  • Good communication and interpersonal skills, with willingness to work with people from diverse backgrounds.
  • Good command of both written and spoken Chinese (Cantonese and Putonghua) and English.
  • Proficient in PC skills, including MS Word, Excel, PowerPoint and Chinese word processing.

Contract Period:

1 year (Renewable)
