Job summary

TOUCH Community Services in Hong Kong seeks a supervisor to coach a team of Care Associates delivering Enhanced Home Personal Care (HPC+) services, tailored to each client’s needs. You will coordinate with internal teams, caregivers and external partners to monitor well-being and improve care outcomes.

Responsibilities include staff management, client care assessments and collaboration with multidisciplinary teams to plan, document and monitor care plans, ensuring compliance and timely reporting.

Qualifications

• Diploma/Bachelor’s in Nursing/Social Work/Allied Health, or related field.
• Experience in healthcare, eldercare, or community care with supervisory responsibilities.
• Strong leadership, communication, and problem-solving skills.
• Familiarity with Singapore’s community care sector and regulations.

Responsibilities

• Identify capability gaps and ensure staff receive relevant training.
• Monitor staff performance, provide timely feedback, and initiate improvements.
• Conduct InterRAI Check-Up assessments and six-monthly reassessments.
• Collaborate with the multidisciplinary team to monitor client progress and needs.
• Coordinate with partners to support clients and document care plans.
