St. James' Settlement in Hong Kong is seeking a Care Coordinator to provide service and operational support across the Core Centre and Satellite Centres, assisting elderly clients in accessing health services and coordinating appointments.
The role supports nursing staff with clinical and non-clinical tasks, handles enquiries, elderly registration, follow-up arrangements, and maintains accurate records, working within a multidisciplinary team to deliver compassionate, compliant care.
Completion of HKDSE with Level 2 or above in five subjects (including Chinese and English), or equivalent qualifications. Relevant training in healthcare services will be considered an advantage
Degree holder with 2 years relevant post-degree experience or Associate Degree with 5 years relevant experience is preferable
Minimum of two year’s working experience, preferably in elderly care or related services is preferable
Strong communication and interpersonal skills, with sensitivity to the needs of elderly clients. Possession of blood-taking skills will be given priority
Ability to work effectively and collaboratively within a multidisciplinary team
Mature, detail-oriented, able to work independently, and demonstrates a systematic approach to work

In 1987, the headquarters was opened in Stone Nullah Lane so that the scope of services could be further extended.
