A leading healthcare provider is seeking a Stop Smoking Service Facilitator to engage patients ahead of surgery and deliver high-quality smoking cessation interventions. The role involves collaborating with a multidisciplinary team, managing a caseload, and providing training to health staff. Candidates must be trained stop smoking advisors with experience in health improvement programs. This is a fixed-term position for 12 months.

This exciting new role will contribute to the delivery of the LGT Stop Smoking Service objectives and the wider Trust objectives by providing high-quality stop smoking interventions to patients on the elective waiting list at both Lewisham and Queen Elizabeth (Greenwich) Hospitals.
This role will be working across the acute and community settings, as well as liaising with the population health team.
As a part of the stop smoking team, you will work with both the inpatient tobacco dependence treatment team and the community stop smoking service to help individuals from the CORE 20plus5 group to stop smoking.
Contract will run for 12 months from the start date.

The Stop Smoking Service Facilitator will proactively engage with patients prior to their surgery to screen their smoking status and to engage them into a stop smoking intervention as a part of a multidisciplinary team.
Linking closely with the specialist advisors in Lewisham and Greenwich, they will monitor, track, and record the progress of the smokers on this pathway, as well as managing a small caseload of quit attempts.
They will also be tasked to maximise referral opportunities from the surgical pathway to ensure high quality stop smoking interventions are delivered by all health professionals at every opportunity.
The post holder should be a trained stop smoking advisor with experience delivering training to a range of professionals who also has experience of working in the acute setting.

Job Summary:
Accountable to Stop Smoking Team Lead. The post holder will play a key role in supporting patients identified as smokers from the elective waiting list who are in the Core 20 plus 5 group. They will work within the Trust and across the community, taking responsibility for identifying smokers, booking appointments, providing follow up, and delivering stop smoking support whilst also reviewing the elective pathways and providing training and resources for key stakeholders.
The post holder will have passed the NCSCT practitioner assessment and will be competent to provide effective patient care in a range of settings, having a close regard for relevant Trust policies to ensure safe and effective working practices when working with vulnerable patients, maintaining patient confidentiality, and designing individually tailored support programmes according to evidence-based practice for the effective delivery of stop smoking interventions throughout the elective pathway using the standards laid out in NICE guidelines (NG209) and by the NCSCT.
They will liaise closely with the community stop smoking services and will be responsible for developing referral pathways and maintaining close relationships with the services to track patient outcomes to ensure they receive a high standard of care to maximise their chances of stopping smoking.
