NHS Scotland is seeking a motivated Social Worker to join the Joint Transitions Team in Inverness on a part-time basis (21 hours per week). The team supports young people aged 14 to 25 transitioning from children to adult services across NHS Highland South and Mid area.

Applicants should be registered with the SSSC and hold an appropriate Social Work qualification, with knowledge of relevant Scottish legislation.

Formación

• Registered Social Worker with the SSSC.
• Understanding of Adults with Incapacity (Scotland) Act 2000.
• Knowledge of Adult Support and Protection and Self Directed Support.

Responsabilidades

• Participate in the team duty service.
• Undertake assessments, care planning, reviews, and risk assessments.
• Carry out statutory social work duties.
• Work with young people, families, and multi-agency colleagues.
• Support effective transition planning and service development.
