Job summary

NHS Highland invites applications for a Social Worker to join the Joint Transitions Team in Inverness on a part-time basis (21 hours per week). You will carry out assessments, care planning, reviews, and statutory duties, working with young people aged 14 to 25 as they transition from children’s to adult services across the South and Mid area.

Strong communication and collaboration skills are essential. You should be registered with the SSSC as a Social Worker and have knowledge of the Adults

Qualifications

• Registered with the SSSC on the Social Worker part of the Register.
• Knowledge of the Adults with Incapacity (Scotland) Act 2000 and understanding of Adult Support and Protection and Self Directed Support legislation.
• Strong assessment and communication skills; capable of handling a varied workload.

Responsibilities

• Participating in the team duty service.
• Undertaking assessments, care planning, reviews, and risk assessments.
• Carrying out statutory social work duties.
• Working in partnership with young people, families, and multi-disciplinary colleagues.
• Supporting effective transition planning and service development.
