NHS Highland South and Mid Area is seeking a Social Work Assistant Practitioner to join the Joint Transitions Team in Inverness. The role supports young people aged 16-25 as they navigate the transition from children’s to adult services, working autonomously and within a multidisciplinary team.

Responsibilities

• Assess and respond to the needs of young adults aged 16-25 years.
• Contribute to planning, monitoring and review of support arrangements.
• Work closely with individuals, families, independent sector providers and partner agencies.
