An exciting opportunity has arisen for a motivated and enthusiastic Social Worker to join the Joint Transitions Team on a part-time basis (21 hours per week).
Based in Inverness, the team supports young people aged 14 to 25 years as they transition from children's to adult services across the NHS Highland South and Mid area. We work collaboratively with young people, families, health professionals, education services, and partner agencies to ensure person-centred and effective transition planning.
The successful candidate will be able to work both independently and as part of a busy, supportive team. The role involves contributing to the team's duty system, undertaking assessments, care planning, and reviews, and carrying out a range of statutory social work responsibilities. You will have strong assessment and communication skills, be able to manage a varied workload, and demonstrate a commitment to supporting young people to achieve positive outcomes. You should be registered with the Scottish Social Services Council on the ‘Social Worker’ part of the Register and hold an appropriate qualification that is recognised by the SSSC. In addition the candidate should have a good understanding of the principles of the Adults with Incapacity (Scotland) Act 2000 and a knowledge of the Adult Support and Protection and Self Directed Support legislation.
This is an excellent opportunity to join a dedicated team working with young people with a range of needs, including learning disabilities, autism, and complex health needs, during a key stage of their lives.
If you are committed to person-centred practice and enjoy working within a collaborative, multi-disciplinary environment, we would welcome your application.
