Zusammenfassung

South Gloucestershire Council is seeking a dedicated practitioner to join the 0-25 Disabilities Service. You will support children and young people aged 10-25 to develop independence, build confidence and access opportunities in home and community settings.

Working with families, you will assess and review care plans, liaise with health, housing and education partners, and contribute to safeguarding. A professional qualification and experience with complex needs are expected, with DBS clearance

Qualifikationen

• Hold or be working towards a professional qualification in any discipline such as an NVQ 3 or equivalent.
• Experience of working in similar service areas with children and young people, especially with complex needs and challenging behaviours.
• Knowledge of safeguarding, DoL's and the Local Offer is desirable.

Aufgaben

• Assess, monitor, evaluate and review cases and care plans against outcomes.
• Maintain confidential reports and records in line with guidelines.
• Provide advice, support and guidance to children, young people and families.
• Regularly liaise with external providers e.g. health agencies, housing and emergency services.
• Promote the Local Offer and liaise with networks including social workers and schools.
• Act in accordance with safeguarding procedures to keep vulnerable people safe.
