Job summary

South Gloucestershire Council is seeking a committed practitioner to join the 0-25 Disabilities Service, supporting children and young people aged 10-25 to develop independence and reach their potential. You will work with families, build confidence, and help with transitions during key life stages within home and community.

This role requires relevant qualifications and experience with SEND, safeguarding, and DoLs knowledge.

Qualifications

• Hold or be working towards a professional qualification (NVQ 3 or equivalent).
• Experience of working in similar service areas with children and young people, especially with complex needs and challenging behaviours.
• Ideally, knowledge of safeguarding, DoLs, Local Offer and related regulations.

Responsibilities

• Assess, monitor, evaluate and review cases and care plans against identified outcomes.
• Maintain confidential reports and case notes, and record care plans and risk assessments.
• Provide advice, support and guidance to children, young people and families.
• Liaise with external providers e.g. health services, housing and emergency services.
• Promote the Local Offer and liaise with social workers, health professionals and schools/colleges.
• Act in accordance with safeguarding procedures to keep vulnerable people safe.
