Building positive relationships with individuals and their families, you will provide tailored support that helps them navigate key stages of their lives and access the opportunities available to them. Some of the children and young people you work with will have special educational needs and disabilities (SEND).

You will join a team focused on early intervention and supporting successful transitions for children and young people referred to the service. Through person-centred support, you will help individuals build confidence, develop life skills and work towards greater independence, ensuring they receive the right support at the right time.

What you will be doing
  • It will be your responsibility to assess, monitor, evaluate and review cases and care plans against the identified outcomes.
  • You will maintain confidential reports and case notes and record care plans and risk assessments in accordance with professional and council guidelines.
  • In a strength-based, person centred approach, you will provide advice, support and guidance to children, young people and their families.
  • Ensuring effective communication, you will regularly liaise and work with external providers e.g. health agencies, housing and emergency services.
  • Working with families, you will promote the Local Offer and liaise with their networks which could include social workers, health professionals, schools/colleges and colleagues in our EHCP team.
  • You will act in accordance with safeguarding procedures to keep vulnerable people safe.
What we need from you
  • We require you to hold or be working towards a professional qualification in any discipline such as an NVQ 3 or equivalent.
  • It is essential that you have experience of working in similar service areas with children and young people, especially with those who have complex needs and challenging behaviours.
  • Ideally, you will be able to demonstrate knowledge of relevant legislation, regulations and codes of practice, e.g. safeguarding, mental capacity/Deprivation of Liberties (DoL’s), the Local Offer.
What you need to know
  • You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
  • Business insurance will need to be added to your current insurance policy.
  • Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) for both Children and Adults is obtained for this post.
