Early Break, a youth charity in Rochdale, is seeking a Young People’s Specialist Substance Use Worker (full-time, 37 hours). The role focuses on one-to-one casework with young people facing substance use and related risks, potentially involving evening or weekend sessions.

You will assess needs, build SMART care plans, and work in community settings to coordinate care with families and partner agencies. You will deliver structured interventions, help with referrals to in-patient treatment when

Competenze

• Deliver 1:1, face-to-face work with young clients and parents/carers as appropriate, including evening/weekend sessions.
• Complete a full assessment of each client’s needs and develop, monitor and review SMART care plans.
• Offer structured treatment interventions, including substitute prescribing, if required and appropriate.
• Engage in community-based settings to engage and deliver services to potential clients.
• Liaise with partner agencies to meet treatment needs of referred clients.
• Record and report all required client information using the company’s database and data protection rules.
• Keep knowledge up-to-date on issues affecting young people through training.
• Contribute to charitable activities including social media and fundraising.
